The bald spots may be because he is allergic to fleas. I know cats have this problem A LOT when they get fleas and dogs can have it too. When we had a flea problem, Smokey had actually scratched himself bald in a couple of areas, so that could be it too. Definitely consult a vet.
As for weight... is his poop normal? The reason I ask is because when my rescue yorkie came to me he had severe anxiety and anytime we would put him by himself, even if he could see us (like if we were in bed), he would get diarrhea. After we let him free roam in our master bedroom with our other yorkie when we were gone, his diarrhea stopped and he gained weight. He was a little over 7 lbs when we brought him home, now he's 14 lbs (he's really tall and long) and a good weight. If everything seems normal you could try and mix some wet puppy food with his regular dog food because it's higher in fats than adult food and maybe feed more often, but consult your vet because he/she can give the best advice for this and would be able to tell you what a good weight for your dog is.
