Does she step in her poo even when she poos outside say on the lawn? At four months, she's certainly ready to be working on doing her potty outside.
My girls are like clockwork, they either wake up and do a poo in the am or immediately after their breakfast or dinner...sometimes they actually will all be on the same routine and then other times everyone is doing it at different times. So, if you can try observing what time she usually does her poo, try and plan to take her out "before" so, she get's used to doing it outside.
The only time I leave piddle pads out is after we all go to bed...for those whoopsie times during the middle of the night.
