Just as everyone is saying: go back to the beginning.

We never use pee pads (I can't stand the thought of them pooping in the house AT ALL!). I use a crate (a small one) and when he was NOT being supervised continually, he was in it. And we went out every hour on the hour. Lots of praise, some treats (he isn't big on treats!), but the real issue is training YOURSELF to be vigilent and take him out and stay with him (can't just open the door and let him go with the other dogs).

That will probably be the only way. Or, buy a playpen and use pee pads in that when you aren't around.
