First i'd take her to another vet and have that lump aspirated. They need to stick a needle in it to get some cells out. Then the vet will look at them under the microscope to determine what kind of lump it is. That is a decent sized lump under her chin and i would have that taken care of right away and it will probably need to be removed.
As for the skin, ask the new vet about that too. it could just be dry skin or it could be an infection.
