I'm kind of thinking it's not that since the vet has already diagnosed an ear infection and treated it. My guess is dry skin. When you bathe him make sure you rinse really good all over. It could very well be the food he's eating too. Food makes all the difference in yorkies and their coats, joints, teeth, bones, digestive etc. If you do switch foods don't switch it on him cold turkey, you have to mix the new in with the old untill the old food is gone to avoid tummy upset. I'd get some shampoo that's safe for puppies, usually tearless is pretty safe. If you go into the YT groomer section and ask I'm sure someone will have some names of good shampoos to try.
