The leaving the food down and the dog will eat rule certainly does not always apply. With a 4 pound dog not eating for more than 24 hours and then the owner noticing lethargy, this system is not going to work. Karo syrup is fine but that can't be a constant thing because it really should be balanced with protein.
I have no idea what I'd do in your case, but if my 4 pound dog was lethargic, I would absolutely not let him/her decide when to finally eat.
