No, I was referring to people who choose to ignore their vet's advice, and believe that their vet is just an opportunist, who would sell them a product that they don't need. I find a vet I can trust and go by what my vet says, here's a map that gives the incidence of heartworm in California. U.S. Regional Map of Lyme disease, Canine Ehrlichiosis, Heartworm, Anaplasma. As you can see, it really varies by counties.
