Some vets do give the last set at 12 weeks but it is not a good schedule at all. I think I'd wait til 13 weeks and then do one more at 17. Sixteen weeks is really the earliest most peopel consider stopping and that is a bit young. Eighteen weeks is probably better. Starting at 6 weeks just interferes with maternal antibodies leaving the maternal antibodies and the vaccine not too effective.
Unless you are ordering from a trustworthy company and know exactly how to give vaccines, it is really better to just let the vet do it.
