I looked at the form and it seems fine to me. I think maybe some people just don't like paperwork. A couple of other thoughts:
I don't have a fenced yard, but then my dog is never off a leash.
A new pet owner might not have a vet reference.
I had two Yorkies and a cat die within a year and a half from old age.
