I wouldn't be offended at all. If anything, it would reassure me as to the quality of breeder I'm dealing with.
Also, I would think as a breeder I would want one (not that I am a breeder). It takes time and thought to fill one of those out so it should help to identify those serious about getting a new pup.
I think you should keep your questionaire. Those who don't want to take the time to fill it out don't deserve one of your puppies anyway, IMHO.
