It's pathetic, but YES he needs to keep it on. I remember when I got my cat fixed, he had the cone on, and it was too big. As soon as he took a step forward, the edge of the cone dug into the carpet, and his head ended up down towards the carpet, cone down as well. It was funny for us, but not for him. In the long run, it's for his betterment. Just keep and eye out. There are soft cones out there as well. Look for it in your pet shops.
