I can understand your frustration. Is this while she's confined to a small area while you're gone? If so, she's probably really bored. Do you have puzzle toys and such in with her? This is really normal puppy behavior. I would make sure that the area can be cleaned easily. I would also look at how long the pup is left alone during the day. Maybe a puppy sitter would help.
If it's happening while you're home, my best advice is to wear her little butt out by taking walks and playing. Just play and play and play. Also, change the pad every time she goes on it and then praise her and distract her from it by playing with her.
