That's another thing, I HAVE caught her midstream. She knows she's doing wrong otherwise she wouldn't run upstairs before I even tell her bad girl.

She'll be 2 in July and she's been consistently using the pads since she was around 7 months old. She would have an accident maybe like once in a month but that was it.

I haven't changed anything, everything has been going the same. Her pads stay in the same spot and when she peed on the floor the first time, I got onto her just like I would if she had peed on the carpet before.
