Whether for spaying or future dental etc. you can't avoid putting them under, unfortunately. I have never experienced any problems with any surgeries in my 35 years of owning Yorkies. I have had three very small ones done in the last two months. One was just done yesterday with no problems. Baby teeth were pulled at the same time in each case. Arielle had fourteen baby teeth pulled and spay and she weighted at nine months old at 2.8 pounds. The next say you couldn't hold her down. Unbelievable! I am nervous every time they go under. I cannot stress enough, know you doctor!!!!! Good Luck.
