died as in the heart stopped or died as in he stopped breathing? these are two different things that people get confused.

The dog could be in picture perfect health and still have complications during surgery. They could be allergic to the anesthesia or it could be human error. There should always be a anesthesia technician there to monitor the dogs vitals while under. The vet cannot do this and surgery at the same time.
