It's your job to find out the reason. I'm assuming the peeing isn't marking and is actual peeing, but maybe he couldn't hold it long enough and couldn't get anyone's attention. Sometimes, owners ignore pleas to go outside, and the dog gives up. I don't agree with wearing a diaper, I think this teaches them nothing. Some dogs have to wear diapers because they are incontinent, but for a normal healthy dog, it really is a mistake, unless you are going visiting where an accident would be deadly! You say in the title, "It's back to wearing a diaper," so it sounds like you have done this before. You never want your dog to get accustomed to sitting in his on waste. You need to be consistent in your training. It's great if he lets you know, he has to go outside, but you have to ask him several times a day, or just take him outside. Keeping a diary might help you discover why he's having the accidents, and when he needs to go outside. I agree with the others, a pee pad might be helpful, if you are not there to take him outside. Training them to go outside, and training them to notifiy you are two different things, and so this takes more time. If he had a doggy door, he might never make a mistake.
