I'm sorry. Potty training is really tough! We've been there. By the way, your dog is adorable.
This is what I did with Thor: I put him in his crate and took him out every hour and put him on his pee pad. If he didn't go after five minutes, I put him back in his crate. He held it for forty hours the first time, and some ridiculous number the next time, but after that, it really clicked and he's improved steadily from there. By the way, I did go to sleep during those forty hours, so part of the time was overnight.
Basically, I would set up a situation where she cannot fail. If you want her to go outside, you can use the above method, but carry her outside to potty. I would not let her paws touch the floor, because she will probably seize the opportunity to pee. I don't know why she is so determined to go on your floors, but it sounds like you need to restrict her movement until she understands where she is supposed to go.
Also, are you using an enzyme cleaner on her messes? If not, she probably smells where she has already gone, and her instincts tell her to return there. You can use this to your advantage. Next time she has an accident, blot her pee with a pee pad or something absorbant. Then, when you take her out, bring the pee pad (you can put it in a plastic baggy for storage), and have her sniff it. Smelling her pee should encourage her to go.
Also, vigorous exercise usually opens the floodgates, so if all else fails, take her on a quick jog around the block and see if that loosens her up.
