Small, unspayed females come into heat anywhere from six months to over a year for the first time. Heat lasts about 21 days. This will happen every 4-7 months generally.
They can get lethargic and the more heats they go through, the more chance that they get pyometra. Please read about the benefits (and risks) of spaying. If you do it now, she will have a less than .5% chance of getting mammary cancer. If you wait until after her second heat, it goes up to 26%.
A four pound dog shouldn't be bred, so she needs to gain some weight for it to be safer. Please visit the breeder's section on YT. There are good threads there about how to prepare to have a pregnant dog and raise puppies.
