I buy onesies for Puddin', then I modify them.
First, I sew a piece of ribbon into the back neck of the onesie. parallel to the neckband, making a casing. Then I thread some elastic into the casing and stitch ends, making the back neck of the garment elastic (this will ultimately be the throat when it is on the dog).

Then I tack the lapped shoulders down so they don't separate any more, stabilizing the shoulder.

Then I make a small ribbon loop and sew it to the front centre of the onesie (which will ultimately be the back of the neck when it is worn).

Finally, I cut two 8 or 10 inch pieces of ribbon, each of which I firmly stitch to a shoulder. The free ends of the ribbon are threaded though the loop.

I pull the garment over Puddin's head, put her front legs through the arms, then pull the ribbons and tie them in a bow at the back of her neck. This effectively snugs up the neck so she can't walk out of it.

Then I snap the two outside snaps, leaving the middle one open for a tail hole. By putting an adhesive-backed panty liner in the crotch of the onesie before closing the snaps, I get a pee-proof, escape-proof garment!
