I was wondering about pancreatitis, a couple of things you said, made me think of this. First, you have been trying to put on a few pounds, and sometimes with a thin dog, people are more likely to give fatty foods. Fatty foods can cause an attack of pancreatitis, that is a very painful disease and the dog will display odd symptoms. Also the age at which it strikes many dogs is around 5 years. So my questions to you would be, what do you feed, and does she get many table scraps? It's often very hard to diagnose these problems without lots of expensive tests, I wish you luck in finding the problem, but I just thought you might want to rule out any food related problem first. On a side note, had she been given any Pepto Bismol lately, as this will cause the feces to look black. I wasn't sure if the vet actually checked it for blood, or just did a visual.
