Sometimes a puppy will be born with the cord too tight and still inside the mom. I hate it when that happens. I break the sack and dry the pup and just wait a little bit for the placenta to release some. You just have to make sure the mom stays calm and does not jerk around too much. I will also reach up into the vulva and gentle pull on the cord to get more slack. As long as the sack is off the pups head he will be just fine for a little while.
