Meg went through the exact same thing when she was 4 mo. She would be unsure about being on the leash, and going outside? Forget about it. She would sit at the door and cry. The goods news is that she quickly outgrew this phase. Bring a favorite toy outside with you so she can have something familiar. And give her some treats. To convince Meg to go outside, I would put a treat right by the door, then right outside the door frame, then a little further out, etc, until she was going out on her own. She would dart out, snatch the treat, then dash back inside, but at least she was learning that it was okay to be outside. Just take little steps. Let your dog get used to the front porch before you take her onto the steps, grass, cement, etc.
As for getting her used to being on the leash, she will get used to it. I'd put Meg's leash on, then step a few feet away and squeak an irresistable squeaky toy. Meg couldn't resist investigating and would come explore. Then I'd move a little bit further away, etc. Just be patient. You don't want to introduce your pup to too much at once (and risk having a nervous, anxious dog).
It took Meg probably 4 days before she was comfortable going in the hallway (I live in an apartment). Then it took another couple of days before she was comfortable walking on different colored tiles, and another day before she was comfortable going in the elevator. Baby steps! Of course, now Meg can't get enough of the hallway, elevator, outdoors, etc.
