I just wonder if the leakage is due to feces being somewhat blocked so the liquid gets through and leaks out. Could also be some nerve damage due to the past constipation or current straining. Constipation can lead to mega colon - a very large colon. I still wonder about him having stenosis, a narrowed area in the colon. It would cause constipation and straining and pain I would think. It's a congenital defect that happens during development. Have you done xrays? Diagnostic images or exploratory surgery may be a good idea
