I'd say possibly worms.
Also, it is possible to have a pin-hole sized cleft, that you can't really see in such a tiny mouth. That's what my Zaccheus had, and as he grew, it turned into a full mid-line cleft.
I bottle fed him every hour to two hours for several weeks, kept him on a heating pad, and under a light, or sometimes in a pouch next to my heart.
