Every 3-4 weeks...never 2 weeks apart.
8, 12 and 16 weeks is okay but I'd prefer to start later if I thought that I could keep the pups safe. I would not do any before 8 weeks. You would just be interfering with maternal antibodies anyway (which would probably happen if you started at 8 weeks also but not everyone is comfortable with the 12 week start...).
