I was going through the same thing with night time accidents. I was ready to pull my hair out too. Chloe came with her own crate from the breeder.

Well I read somewhere that if they are having accidents in their crate, that they have too much room and you need to downsize it, they won't soil where they have to sleep.
So, I put an old throw pillow in her crate. She has enough room to lay down and that's it. Since I did this we have not had any accidents. I have her crate next to my bed. She whines when she needs to go out. It helped us so much!!!
