If they really don't have small dog experience or you aren't comfortable, move on.

What anesthesia do you use for maintain?
If you aren't open 24/7, what do we do in an emergency?
Will you alter your vaccination protocol and not force me to give them yearly?
How long have you been practicing?
Is surgery done on a heated surface?
Is an IV placed before surgery?
Is preop blood work offered?
How do you monitor during anesthesia?
Do you do the surgery alone or is somebody (a tech) there to help?
Do the techs anesthetize without the vet in the room?
Are they willing to write a prescription for safe products that they do not carry or that are too pricey?
Is the office very clean?
Will I be forced to leave my dog overnight after surgery if there is nobody here to watch them?
