I would definately spay her. She has probably had at least 1 or 2 heats already and the risk or cancer increases with each heat. I bet she'll eat much better after her teeth are cleaned and the baby teeth pulled. Yes, there are some down side to spaying but they are much more rare then the risk of cancer. I don't know what health issues the above poster is referring to but if her only problem is being underweight she shoud do fine.
