Thanks for the replies. I like the bell idea, although in this case I KNEW she was coming up behind me and we just misjudged each other. My 9 year old was actually right behind the puppy and blames herself for not foreseeing it and picking Eliza up. We had some tears and will have to keep figuring out ways to keep the puppy safe.

On a positive note, the Vet said that Eliza does not have an ear infection as she had previously thought. They were just really dirty--she cleaned them out. The blood in her stools seem to have been caused by her pain medication, which she doesn't appear to need at ALL and so I have discontinued. They added a splint to her leg wrap--just to be on the safe side and I think Eliza is on the upswing.
