He needs to have it removed...oral surgery.
They may do xrays and pull other ones also.
With the plaque build up and risk of bacteria spreading to his heart, liver, kidneys and lungs, it shouldn't be let go.
Sometimes antibiotics are given before surgery to keep infection risk down and they should be given after.
