Approximately 83 vouchers will be made available each hour of each day beginning at 9:00 a.m. ET and ending at 9:00 p.m. ET on a first come basis. If the supply of vouchers for any given hour has been exhausted, you may visit the website each hour thereafter (between 9:00 a.m. ET and 9:00 p.m. ET) and follow the directions provided until such time as you receive a voucher or the Offer Period has expired.
To redeem voucher and receive your $20 check by mail, you must take your dog or cat to a licensed veterinarian between February 16, 2009 and May 31, 2009 for a preventive care visit (e.g., vaccinations, wellness exams, flea and tick treatment and prevention, heartworm preventative, dental care etc.). Veterinarian must be licensed with the state in order for you to receive the $20 check. Voucher recipients must complete the voucher by providing his/her first and last name, type of pet (cat or dog), pet's birthday (month/day/year), phone number, address and email address. Veterinarians must complete the following information on the voucher: first and last name, name of clinic, clinic address, clinic phone number, email, date of preventive veterinary visit and signature. Employees of government entities are not eligible.
