A good breeder doesn't just know the parents, he know the grandparents and the great grandparents. That why dogs have pedigrees, so breeders can check the family tree. A 5 pound dog only gives you so much information, you want to know if he was the biggest smallest or average of his relatives. Once a breeder knows the line, she then even has a better idea, with the birth weight, and the weight in following weeks. Runts of the litter sometimes can be the largest dog when full grown, so it's best to consider all the factors when trying to estimate adult weight. So you could call it an educated guess.

By the way, nearly all byb's say the dogs will be 4-5 pounds, they know this is what people want.
