That's understandable but she should have a better chance at living a long life with the surgery. Have you thought about Care Credit? Are you sure it isn't MVD instead of a shunt? That would be inoperable. Since she can't have surgery right now, is she on a special diet and supplements?
A two pound dog can have surgery for both LS and spay with the correct anesthetic protocol although the only place I'd consider having it done is University of Tennessee or maybe Cornell. Of couse the bigger the better though and I would be very nervous about putting a two pounder under also.
The milk thing happens occasionally and some dogs who are having a false pregnancy even strain to develop their "puppies".
