I see that you've done tests -- but has anything been grown out in culture media? I mean, I think the areas really need to be cultured and grown out, so the "bug" can be identified. Of "bugs" (generic term), it could be Staph, Candida, Ring Worm, etcetera. Pyoderma? Mange?

Without an actual diagnosis, I'm at a loss of ideas for you.

It is really awful that not even steroids work.
