Try blotting a pee pad with some of her pee the next time she goes outside. Then, when she's held it all day and is dying to go, put her on the pad and hold up the smelly corner to her nose. Her instincts may take over and release the flood gates. Then of course, praise like crazy and give her a really good treat.

Something else I've been thinking about it how to differentiate the pee pad from other papers and the rug. Dogs don't have very good color vision, so if you're messy like I am and have work papers on the floor, it may be confusing.
