Well I've heard if you don't want him to ever lift his leg you should neuter nim. I would not be worried about his size as long as you trust your vet. My Teddi is 3 lbs and she had had 3 surgerys and come through them just fine. She got spayed, had teeth pulled and had liver shunt surgery. I think there are also more health benefits to getting him fixed.
