I would really try to get this looked into, as it sounds like he is in pain. What premium food are you feeding, exactly, and what foods have you tried in the past? Maybe it is an ingredient that is bothering him.
If for 2 years you were giving mostly just plain chicken and cheese for the diet, his system is likely out of balance quite a bit - and getting him on a proper diet could help a lot.
