With dogs, usually the smaller the dog the longer baby teeth can take to fall out. With the toy breeds, baby teeth ought to be out by 6 or 7 months of age. If yours is 10 months, most likely those teeth aren't going to fall out on their own. You don't want to leave them in as it will affect the bite and allignment of the teeth, and tarter will build up quickly between those teeth. I personally would not have an intact pet unless there was a major health issue preventing the surgery. As I see it, there is no reason not to spay/neuter so why not do it? Be sure to have bloodwork done beforehand to check for any underlying health issues and have an experienced vet, and I'm sure all will be fine. He'll be back to his normal self within a few days.
