This is a great idea. I would also include a section on the application for references (the family's vet, a personal reference and a neighbor) and verify with all 3 before I would even consider talking further with a buyer. There are always so many horror stories that I definitely agree with everyone that its better to be safe than sorry.

I haven't yet taken the jump to breed my first litter--thank goodness--because knowing me I would DNA test and background check potential puppy buyers!!
