what kind of flea/tick shampoo are you using? once a week seems over load for a little puppy like that, and it might be why she is scratching, since her skin is probably so dried out! Did the vet find any fleas on her? any flea dirt? I would ask them to do a skin scrapping and see if she has mange or any other form of skin disease, or bacteria. did she get into anything outside!?
