It's normal for the placentas to not come right out with the puppy.

So what I do is, use 2 sets of clamps. Put one about 1/4 away from the puppy's belly on the cord. Then another a little farther from that, towards the mom. Leaving enough room to get scissors in there to cut. Or you can just tear it with your fingernails. Leave the one on the mom and you can use it to keep track of that placenta. That way, she doesn't suck the cord and all back up in there.

If you decide to pull on that cord to help the placenta come out, you have to do it very carefully. If you do it too fast and too hard, it can tear and will leave some in there. So, just pull very gently. Mostly just putting a little pressure on. If you don't feel it start to give then just wait a while.
