I'd really be concerned about a young pup having this severe diarrhea - he could really go down fast.

I would get him back in asap and check for giardia. Giardia is notorious for hiding under microscope, so it may have been missed the 1st time. Also, it is very re-infective - so if he had it, then meds made it better - he probably was reinfected bc you have to clean everywhere when they're healing from giardia.
