Yes, I would insist on meds. I wouldn't let Ellie have abdominal surgery and not give pain meds unless a medical condition prevented it. It is "normal" for some vets not to give them because, "They are calmer without them", "They don't feel pain like we do", "They are fine without them", etc. Then there are other vets who say it is a very painful abdominal surgery and they should have meds. Ellie is liver compromised and her vet still wanted her on something. Dogs can hide pain very well, so you can't be sure she isn't in pain. Meds can also help with swelling. I couldn't imagine having abdominal surgery without pain control...
Not sure about the bunched appearance of the incision but closing with sutures externally is okay. Ellie had staples and everything turned out okay.
I hope she recovers quick.
