How about a Protein C test? This is just a blood test. Normal levels indicate MVD and abnormal indicate a full shunt. There are exceptions though. Then if the number is in shunt range, a scintography would be next. However, you might want to go with scintography even if the Protein C test is normal. Personal preference...
Because he is symptomatic, he will need to be on a specific diet. Has your vet talked to you about that? He will also need supplements.
