I clean them out once a week and they are always dirty, but I think that is because she has too much hair in there and she is going to the groomers to get them cleaned up, but wondering if I should be concerned.

We'll I have actually never heard of this before. But yeah I would be concerned. If you don't see a bump or something she is scratching that is hurting her, then I would take her to the vet. Maybe it's some kind of ear infection.
