Did the vet do a skin scraping for mites? It almost sounds like she may be allergic to grass though - when she's outside, is the on the grass? Allergy tests aren't really reliable, but what's really odd is that only 2 things came back for her - usually the list is about 10 minimum, it seems.

In case it's a food allergy - what are you feeding? If she has atopy (environmental), then she is vulnerable to *all* kinds of allergies (like food or whatever) bc the immune system becomes more vulnerable. So, you do want to choose a food that has the least known canine allergens. Also, it's good you're giving Fish oil (EPA & DHA), but in the case of atopy - you really want to get the fatty acid called GLA in the system. The only oil I've found that meets the right stuff for me is Ark Naturals Royal Coat, which contains fish oil, borage oil (the GLA), and Vit A/D/E.
