You have gotten some excellent advice. I only have one thing to add and that's about your vet visit. When you go to the vet, DO NOT let him touch ANYTHING in the lobby. Carry him in a carrier, take him out only in the exam room, and put him right back in to walk him out. Also, don't let anyone touch him there aside from the vet and the tech that your are dealing with. If he's super tiny and stressed, his immune system will be weak and he could become sick easily. I cannot even imagine having a 5 week old puppy!!!!
