If I understand you correctly, your nan got the dog at 5 weeks; the minimum age that a Yorkie should leave the mother is 12 weeks. Some people think just because a mother dog stops nursing that the puppies can be taken away, this is just not true and important socialization skills are taught be the mother and littermates. Your puppy has never received the socialization it needed from the mother, and probably didn't receive what it needed from your nan, so it will probably be very difficult to socialize it now. You need more advice than I can give you, and it's important when retraining to do everything absolutely correct, or you can make the problem worse. Have you considered a dog trainer? Petsmart offers training on a one to one basis, as well as classes, maybe you could talk to someone there and see if they have a program that could help you. Cesar, the Dog Whisperer, has a TV show, and he has addressed this issue several times, perhaps you could rent his series if you don't get his program.
