I have trained my three to only use the back door to go outside. This way when someone comes to the from door I don't have to fight them. My suggestion would be do everything that the others suggested but to only use the back door to go outside. It's the same in the car, they are only allowed in and out of one door in the car as well. I also make them sit and stay when I open the back door to go outside.
