The vet refused to xray unless you agreed to have surgery done?
I would find a new vet if that is the case.

A dog who has been limping this long needs medical attention and possibly surgery. Why not get a second opinion?

If this is luxating patella, a general DVM may not be able to do the surgery anyway. You would want to see an orthopedic specialist.
